However, in a <a href=\"https:\/\/www.mja.com.au\/journal\/2024\/221\/11\/towards-cure-long-covid-strengthening-case-persistently-replicating-sars-cov-2\" target=\"_blank\" rel=\"noopener\">perspective article<\/a> published in the latest Medical Journal of Australia, we argue that much, if not all, long COVID appears to be driven by the virus itself persisting in the body. <\/p>\n\n<p>Since relatively early in the pandemic, there has been a <a href=\"https:\/\/www.nature.com\/articles\/s41590-023-01601-2\" target=\"_blank\" rel=\"noopener\">recognition<\/a> that in some people, SARS-CoV-2 \u2013 or at least remnants of the virus \u2013 could stay in various tissues and organs for extended periods. This theory is known as \u201cviral persistence\u201d. <\/p>\n\n<p>While the long-term presence of residual viral fragments in some people\u2019s bodies is now well established, what remains less certain is whether live virus itself, not just old bits of virus, is lingering \u2013 and if so, whether this is what causes long COVID. This distinction is crucial because live virus can be targeted by specific antiviral approaches in ways that \u201cdead\u201d viral fragments cannot.<\/p>\n\n<p>Viral persistence has <a href=\"https:\/\/www.nature.com\/articles\/s41579-024-01125-y\" target=\"_blank\" rel=\"noopener\">two significant implications<\/a>:<\/p>\n\n<ol>\n<li><p>when it occurs in some highly immunocompromised people, it is thought to be the source of new and substantially different-looking variants, <a href=\"https:\/\/theconversation.com\/the-emergence-of-jn-1-is-an-evolutionary-step-change-in-the-covid-pandemic-why-is-this-significant-220285\" target=\"_blank\" rel=\"noopener\">such as JN.1<\/a> <\/p><\/li>\n<li><p>it has the potential to continue to cause symptoms in many people in the wider population long beyond the acute illness. In other words, long COVID could be caused by a long infection.<\/p><\/li>\n<\/ol>\n\n<h2 id=\"what-does-the-research-say\">What does the research say?<\/h2>\n\n<p>While there remains no single study that confirms that persistent virus is the cause of long COVID, collectively several recent key papers make a compelling case. <\/p>\n\n<p>In February, a <a href=\"https:\/\/www.nature.com\/articles\/s41586-024-07029-4\" target=\"_blank\" rel=\"noopener\">study in Nature found<\/a> a high number of people with mild COVID symptoms had extended periods of shedding the genetic material of the virus, so-called viral RNA, from their respiratory tract. This study found the risk of long COVID (measured four months following infection) was higher in those with persistently positive viral RNA. <\/p>\n\n<p>The <a href=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(24)00171-3\/fulltext\" target=\"_blank\" rel=\"noopener\">same study<\/a> also gave clues about where in the body the persisting virus resides. The gastrointestinal tract is one site of considerable interest as a long-term viral hideout. <\/p>\n\n<p>Earlier this week, further <a href=\"https:\/\/www.clinicalmicrobiologyandinfection.com\/article\/S1198-743X(24)00432-4\/abstract\" target=\"_blank\" rel=\"noopener\">evidence<\/a> of persistent virus increasing likelihood of long COVID has been published as part of the <a href=\"https:\/\/recovercovid.org\/about#whatIs\" target=\"_blank\" rel=\"noopener\">RECOVER initiative<\/a>, a collaborative research project that aims to address the impacts of long COVID. <\/p>\n\n<p>However, formal proof that virus capable of replicating can last for years in the body remains elusive. This is because isolating the live virus from reservoirs inside the body where the virus \u201chides\u201d is technically challenging. <\/p>\n\n<p>In its absence, we and <a href=\"https:\/\/www.science.org\/content\/article\/long-covid-trials-aim-clear-lingering-virus-help-patients-need\" target=\"_blank\" rel=\"noopener\">other<\/a> <a href=\"https:\/\/www.cell.com\/cell\/fulltext\/S0092-8674(24)00886-9?_returnURL=https%3A%2F%2Flinkinghub.elsevier.com%2Fretrieve%2Fpii%2FS0092867424008869%3Fshowall%3Dtrue\" target=\"_blank\" rel=\"noopener\">scientists<\/a> argue the cumulative evidence is now sufficiently compelling to galvanise action.<\/p>\n\n<h2 id=\"what-needs-to-happen-next\">What needs to happen next?<\/h2>\n\n<p>The obvious response to this is to fast-track <a href=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(24)00436-5\/fulltext\" target=\"_blank\" rel=\"noopener\">trials of known antivirals<\/a> for prevention and cure of long COVID. <\/p>\n\n<p>This should include more left-field therapies such as the diabetes drug metformin. But translating these into forms usable in the clinic is a large hurdle that requires support and investment from governments.<\/p>\n\n<h2 id=\"demystify-and-preventing-long-covid\">Demystify and preventing long COVID<\/h2>\n\n<p>The notion of \u201clong infection\u201d as a contributor or even the driver of long COVID is a powerful message. It could help demystify the condition in the eyes of the wider community and increase awareness among the general public as well as medical professionals.<\/p>\n\n<p>It should help raise awareness in the community of the importance of reducing rates of re-infection. It is not just your first infection, but each subsequent COVID infection <a href=\"https:\/\/www.cdc.gov\/covid\/long-term-effects\/index.html\" target=\"_blank\" rel=\"noopener\">carries a risk of long COVID<\/a>. <\/p>\n\n<p>Long COVID is common and isn\u2019t restricted to those at high risk of severe acute disease but affects all age groups. In one <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.5694\/mja2.52467\" target=\"_blank\" rel=\"noopener\">study<\/a>, the highest impact was in those aged 30 to 49 years. <\/p>\n\n<p><\/p>\n\n<p>So, for now, we all need to reduce our exposure to the virus with the tools available, a combination of:<\/p>\n\n<ul>\n<li><p><a href=\"https:\/\/theconversation.com\/grattan-on-friday-experts-want-albanese-to-lead-on-indoor-air-quality-as-part-of-pandemic-planning-239829\" target=\"_blank\" rel=\"noopener\">clean indoor air approaches<\/a>. In its simplest form, this means being conscious of the importance of well-ventilated indoor spaces, opening the windows and improving airflow as COVID spreads through air. More sophisticated ways of ensuring indoor air is safe involve monitoring quality and filtering air in spaces that cannot be easily naturally ventilated<\/p><\/li>\n<li><p>using high-quality masks (that are well-fitting and don\u2019t let air in easily, such as N95-type masks) in settings where you don\u2019t have confidence of the quality of the indoor air and\/or that are crowded<\/p><\/li>\n<li><p>testing, so you know when you\u2019re positive. Then, if you\u2019re eligible, you can get treatment. And you can be vigilant about protecting those around you with masks, staying at home where possible, and ventilating spaces<\/p><\/li>\n<li><p>staying up to date with COVID booster doses. Vaccines <a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2024.11.19.24317487v1.full\" target=\"_blank\" rel=\"noopener\">reduce long COVID<\/a> and other <a href=\"https:\/\/www.cambridge.org\/core\/journals\/antimicrobial-stewardship-and-healthcare-epidemiology\/article\/effectiveness-of-covid19-vaccine-in-the-prevention-of-postcovid-conditions-a-systematic-literature-review-and-metaanalysis-of-the-latest-research\/A0B115B5D3AA60846799857B801D116E\" target=\"_blank\" rel=\"noopener\">post-COVID complications<\/a>. <\/p><\/li>\n<\/ul>\n\n<p>Hopefully one day there will be better treatments and even a cure for long COVID. But in the meantime, increased awareness of the biomedical basis of long COVID should prompt clinicians to take patients more seriously as they attempt to access the <a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(24)01136-X\/fulltext\" target=\"_blank\" rel=\"noopener\">treatments<\/a> and <a href=\"https:\/\/www.aph.gov.au\/Parliamentary_Business\/Committees\/House\/Health_Aged_Care_and_Sport\/LongandrepeatedCOVID\/Report\" target=\"_blank\" rel=\"noopener\">services<\/a> that already exist.<!-- Below is The Conversation's page counter tag.
